When you attend you need to bring the following:

  • National Insurance and bank details.
  • Name and address, postcode and telephone numbers of 2 referees. One of which need to be from a previous employer.
  • Proof of ID (passport and birth certificate or marriage certificate and proof of address).
  • Work permit/visa (if relevant).
  • 2 passport sized photographs.
  • Any relevant certificates,e.g. Health & Safety manual and handling etc,
  • Criminal Record certificate.
  • NMC Pin card (qualified staff).
  • Record of entry to register.
  • Hepatitis 'B' Vaccination Record.
